Somebody had the lift positioned wrong.
That frame is a worse example of what the "Bellingham Truck's" frame was going to do. The rear axle arches were the worrysome ones.

Al's right, the lift is positioned wrong, but those Mazda trucks are notorious for chassis rust as well. I would bet that the box has been rubbing against the cab for some time now.
One reason I hate to have a car put on a lift if I'm not present. There are too few places that I trust to position the lift correctly. I've had to "instruct" in a very loud and firm voice from the sidelines, where exactly the lift pads should be placed.
This underscores the need to visually inspect your vehicles undercarriage at least on a yearly basis if not more frequently.

All that said, I'm sure most drivers do NOT know the correct lift points for their vehicle.
