What do they do, just bolt them together and mill the sides parallel?

Sorry for the delay
They bolt them together and then bore the centre to match. Hopefully you will be close enough that they do not have to exceed the maximum "ID" of the big end. If it's really close now then it will likely be OK for them to do it. If you have a few other rod ends for them to choose from then take those with you.
